There are many women out there who are interested in breast augmentation, but for various reasons prefer not to increase the size of their breasts using implants. Some women don’t like the idea of having implants in their bodies, are concerned it won’t look natural enough, or are really just looking for a small increase in size.
Whatever the reason, it’s your body, and you have every right to feel however you do about it. If you’re someone who has thought about getting a breast augmentation but was hoping to do it without implants, breast augmentation with fat transfer might be the procedure for you.
Breast augmentation with fat transfer is a procedure in which fat is harvested from other parts of your body via liposuction, and then re-injected into your breasts in order to moderately increase their size, improve their shape, or correct underlying asymmetry. Depending on the amount of fat injected, breast augmentation with fat transfer can increase a woman’s breast size approximately half to a full bra cup size.
Fat is usually harvested from the abdomen, hips, and/or thighs. Your surgeon will begin the procedure by using liposuction to remove fat from any unwanted areas, expertly sculpting and contouring them to your desired silhouette. Your surgical team will then proceed to purify and isolate the desired fat cells for re-injection. The fat cells, known as a fat graft, are then expertly injected into the breasts to ensure that they are correctly integrated into the existing breast tissue.
In experienced surgical hands, breast augmentation with fat transfer is a great procedure in that it allows for a beautiful natural result, while simultaneously treating more than one area of the body: the breasts as well as the areas from which fat is liposuctioned. In addition, the entire procedure uses your own fat to produce the desired result with no need for an implant, resulting in soft and supple, completely natural breasts.
Breast augmentation with fat transfer is also very useful in correcting minor asymmetries of the breasts and because the incisions for fat grafting are very small there is virtually no scarring. Breast augmentation with fat transfer is also an ideal procedure for women that are seeking to improve their cleavage.
Breast augmentation with fat transfer can be performed as an alternative to implants, but at The Plastic Surgery Clinic the procedure can be performed with implants as well.
In some instances there are patients who are in search of a natural look, meaning they would like relatively small implants, but they are also looking for some cleavage. In cases where a patient is naturally lean and an area of concern might be a wider gap between the breasts, implants alone will often not resolve this issue. In these types of cases Dr. Lista, Dr. Ahmad or Dr. Austin, will opt for a fat transfer following implant placement. The harvested fat injected into key areas of the breasts will reduce the gap between the breasts and will equally add some natural cleavage without the need for very large implants.
The advantage of this technique is that it allows for a very natural look and feel by surrounding the implant with the patient’s own fat taken from another area of the body.

With any fat grafting procedure it takes approximately three months in order for your body to fully incorporate the fat in the new area of your body. This means that the final size and shape of your breasts may not be evident for this amount of time. Interestingly, we know that only about 50-60% of the fat that is injected into the breast will survive long term, with the remainder of the injected fat being broken down and re-absorbed by your body.
Everyone’s body is different, meaning that every patient’s result varies from person to person. The best candidates for breast augmentation with fat transfer are women who are looking for a moderate increase in breast size without the need for implants. Typically, you should have some unwanted pockets of fat in the areas of the abdomen, hips and/or thighs from which fat can be removed and harvested in order to inject into your breasts.
Because breast augmentation with fat transfer offers a subtle transformation and improvement of the breasts, the best candidates for this procedure already have a good shape to their breasts, good skin elasticity, as well as minimal asymmetries. Women whose breasts are very heavy or sagging, have excess skin, more visible asymmetries, or are looking for a large increase in size, will be better candidates for other breast surgeries such as breast augmentation, breast lift, breast reduction, or a breast lift with auto-augmentation. In some cases, fat injection can actually be combined with a conservatively sized breast implant to achieve the best overall breast size and shape possible.

Depending on your medical history and the types of medication you may or may not be taking, your surgeon and patient coordinator will outline ways in which to prepare for your surgery, from planning out the proper support system for your recovery at home, to guidelines on what vitamins to take, which medications to take or to avoid, and how to prepare with diet and nutrition.
Patients who smoke will be asked to stop for one month prior to surgery and one month following their procedure, as smoking inhibits wound healing and may actually impact the survival of transferred fat.
The breast augmentation with fat transfer procedure takes approximately 2-3 hours, but can vary depending on the complexity of your case (whether fat is being harvested from more than one area of your body, for instance). Another advantage of breast augmentation with fat transfer is that it is much less invasive than a standard breast augmentation surgery allowing for a speedier recovery and general healing process.
Recovery following breast augmentation with fat transfer typically takes 2-4 days, but we still recommend that patients take at least one week off work and/or from their regular routines in order to allow their body to heal. We encourage gentle exercise such as walking soon after surgery, but strenuous exercise such as running or weight lifting should be avoided for one month following surgery.
Although initial recovery is relatively speedy, your breasts will continue to be swollen for several weeks following your surgery. We always advise patients to be patient with themselves and their bodies. Although you will see immediate results following your breast augmentation with fat transfer procedure, it will take time before the grafted fat is fully integrated into your breasts and becomes a natural part of this new area of the body. In the weeks and months following your procedure you will see your swelling go down and your breasts will gradually become softer.

Though extremely rare, especially in the most experienced surgical hands, there are risks to any surgical procedure. Some risks associated with breast augmentation with fat transfer include: bleeding, infection, skin irregularities (such as unwanted lumps or firmness), persistent pain, fat necrosis, prolonged swelling, complications that may arise from anesthesia, fat absorption, dizziness, scarring (very rare given the small fat grafting incisions), numbness, as well as fat embolism.

The results of breast augmentation with fat transfer are permanent as long as your body doesn’t undergo any major weight fluctuations. If you gain weight, your breasts will increase in size, and if you lose weight, some fat will be lost from the breasts as well. For this reason, it is best to be within 10 lbs of your goal weight at the time you undergo surgery. If you have not had children and plan to in the future your breasts may change as a result of pregnancy and breastfeeding. You can also expect the harvested fat in your breasts to undergo changes as a result of the natural process of aging.
