TERMS & CONDITIONS ELIGIBILITY:
No purchase necessary. Contest is open to all legal residents of Canada (excluding Quebec) who have reached the age of majority in their province/territory of residence at the time of entry, except employees or agents (and those with whom they are domiciled) of (collectively, “The Contest Parties”): The Plastic Surgery Skincare Clinic Inc. (the “Sponsor”); The Plastic Surgery Clinic Inc.; Miracle 10 Cosmetics Inc.; any and all advertising and promotion agencies retained by or associated with The Plastic Surgery Skincare Clinic Inc., The Plastic Surgery Clinic Inc. and Miracle 10 Cosmetics Inc.; and the Contest Judges.
HOW TO ENTER:
During the Contest Period (starting Tuesday, May 7th, at 12:00 AM and ending at 12:00 PM on Tuesday, May 21st, 2019) the contestant must complete and submit a contest ballot (hereinafter the “Contest Ballot”). There is a limit of one (1) entry per contestant. For greater certainty, the contestant may only use one (1) ballot to enter the Contest. Contestants who attempt to enter the Contest more than once by using multiple ballots will be automatically disqualified from the contest and all submitted entries shall automatically be deemed to be invalid. Use of any automated system to enter the Contest is prohibited. Contestants who use an automated system to enter the Contest will be automatically disqualified from the Contest and all submitted entries shall automatically be deemed to be invalid. No Contest entry is valid unless a) it is made through the submission of a Contest Ballot; b) the entry contains all information required by Contest Ballot; and c) the Contest Ballot is submitted and received during the Contest Period. All validly submitted Contest entries (the “Qualified Eligible Entries”) received during the Contest Period will be entered into a random prize draw (see below).
THE PRIZE(S) AND APPROXIMATE RETAIL VALUE:
There is one (1) prize available to be won. Subject to the terms contained herein, the prize shall consist of one (1) CoolScultping treatment (approximate value of $750.00); (the “Prize”). The Prize shall be subject to the terms contained herein; each Contest participant has a chance of winning one (1) prize. The Prize must be accepted as awarded and is not transferable or convertible to cash or credit. No substitutions will be made except at Sponsor’s option. Sponsor reserves the right to substitute the Prize or any component thereof with a prize or prize component of equal or greater value, including, without limitation, but at Sponsor’s sole discretion, a cash reward. Prize will only be awarded to the person whose name and verifiable email address appears on the Contest Ballot.
THE PRIZE WINNER SELECTION:
On May 22nd, 2019, (the “Prize Draw Date”) in Toronto, Ontario at approximately 12:00 p.m. EST. one (1) Qualified Eligible Entry will be selected by random draw from among all Qualified Eligible Entries received during the Contest Period (the “Prize Selected Entrants”). The Sponsor will make three (3) attempts to contact each Selected Entrants by email within ten (10) business days of the Prize Draw Date (the “Prize Notification Period”). In the event that the Sponsor is unsuccessful in contacting the Selected Entrant within the Prize Notification Period or there is a return of any email notification as the deliverable (whichever occurs first), then the Selected Entrant will be disqualified and the next eligible entrant will be selected from amongst the remaining eligible entries. Before being declared the winner, the Selected Entrant will be required to: (a) correctly answer a time-limited mathematical skill-testing question; and (b) sign and return within ten (10) business days of notification, the Sponsor’s declaration and release form confirming compliance with these Rules, acceptance of the Prize as awarded and releasing the Contest Parties and each of their respective officers, directors, agents, representatives, successors and assigned from any liability in connection with this Contest, the Selected Entrant’s participation therein and/or awarding and use/misuse of the Prize or any portion thereof. If the Selected Entrant: (a) fails to correctly answer the skill-testing question; or (b) fails to return the Contest documents within the specified time, then he/she will forfeit the Prize and an alternate eligible entrant will be selected from amongst the remaining eligible entries.

By entering the Contest and accepting the Prize, the Prize Selected Entrant agrees to the publication, reproduction and/or other use of his/her name, address, voice, statements about the Contest and/or photography or other likeness without further notice or compensation, in any publicity or advertisement carried out by the Sponsor in any manner whatsoever, including, without limitation, print, broadcast or the Internet. All entries become the property of the Sponsor, which assumes no responsibility for lost, delayed, incomplete, illegible and/or misdirected entries. This Contest is subject to all applicable federal, provincial and municipal laws. The decisions of the Contest judges with respect to all aspects of this Contest are final and binding on all entrants without right of appeal, including, without limitation, any decisions regarding the eligibility/disqualification of entries. The Sponsor reserves the right to cancel or suspend this Contest, or to amend these Rules without prior notice or obligation, in the event of any accident, printing, administrative, or other error of any kind.
NO WARRANTY OR REPRESENTATION: ALL CONTEST ENTRANTS HEREBY EXPRESSLY ACKNOWLEDGE AND AGREE THAT, IN CONDUCTING THE CONTEST, NEITHER THE SPONSOR NOR THE CONTEST PARTIES MAKE ANY WARRANTY OR REPRESENTATION WHATSOEVER, WHETHER EXPRESS OR IMPLIED, AS TO THE SUITABILITY FOR USE OF OUR PRODUCT WITH RESPECT TO ANY PERSON(S), INCLUDING THE PRIZE SELECTED ENTRANT OR THE EFFECTIVENESS OF SUCH TREATMENT.
COLLECTION OF PERSONAL INFORMATION:
By entering this Contest, each entrant expressly consents to the Sponsor, its agents and/or representatives, storing, sharing and using the personal information submitted with his/her entry only for the purpose of administering the Contest and in accordance with Sponsor’s Privacy Policy unless the entrant otherwise agrees. You may choose to allow this information to be used by the Sponsor and other Contest Parties for promotional purposes (which purposes may include email newsletters and other correspondence containing product information and/or promotions) by selecting the appropriate option upon submission of your Contest Ballot. If you choose to do so but later change your mind, you can opt out at any time.
